Know You'll Get the Care You Want
You can't remove all uncertainty about your future health or the treatment decisions that may become necessary. But an advance medical directive lets you tell your family and your doctors what medical care you want, if a time comes when you can no longer express those desires yourself. An advance directive helps give you peace of mind–you've made your wishes known.

Where to Get Help
If you have questions or need advance directive forms, contact your local hospital or hospice program, your state's medical or legal association, or an attorney. You can also call Eldercare Locator at 1-800-677-1116 to get the name of a local community service agency that may be able to help you. The following national organizations also provide information:
  • Choice in Dying
    800-989-WILL (800-989-9455)
    www.choices.org

  • American Association of Retired Persons
    Legal Counsel for the Elderly
    601 E Street, NW
    Washington, DC 20049
